Park View Senior Health and Living Health Care Center achieved a deficiency-free survey from the Minnesota Department of Health (MDH) in spring 2026. This is the second deficiency-free survey in three years, having previously accomplished the feat in 2024. These results signal that residents receive care that meets the highest federal and state quality and safety standards.
A Significant Accomplishment
This achievement is especially impressive given that Park View was selected for a new Centers for Medicare and Medicaid Services (CMS) survey process for high-performing communities.
“To achieve another deficiency-free survey this year in addition to being selected to this list of high performing communities by MDH is a great recognition of our team that works hard to deliver great care to our residents,” says Park View Health Care Center Administrator Mat Bedard. “It was like hitting two home runs in the same game.”
What is a Deficiency-Free Survey?
The survey followed a comprehensive review of the care and services provided at Park View in Buffalo, MN.
Nursing homes must comply with federal CMS requirements to receive payment under Medicare or Medicaid programs. The state completes regular, unannounced surveys to ensure settings remain in compliance with regulations and provide safe, quality care.
MDH surveyors evaluated the processes, procedures, general operations and physical plants, as well as information submitted by Park View staff. Surveyors also interviewed staff, residents and family members of residents to gain perspective on the care provided at Park View.
The achievement is a testament to the team at the community.
“We are blessed to have the caliber of staff that we do at Park View and our team appreciates having the outside validation that we are providing great care for the seniors we have the privilege to care for,” Mat says.
A Track Record of Excellence at Park View
Park View was also recently ranked No. 5 for Minnesota nursing homes with 50 to 99 beds on Newsweek’s Best Nursing Homes in America list in 2026.
